The primary responsibility of the Child & Family Services Case Manager will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families. 

Specific duties include:

  • Providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies
  • Delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis
  • Providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;
  • Providing services on an as-needed basis both with respect to time-of-day and location
  • Delivering reimbursable hours of service compliant with established standards
  • Completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or the Center's operations in a timely and accurate manner
  • Bert Nash Center requires proof of Covid-19 immunization prior to starting any position. Covid-19 vaccine series must be completed or in process to be cleared for new hire orientation
  • BA/BS degree or four years of equivalent education and/or experience working in the human services field
  • Reliable transportation and automobile insurance; and a valid, unrestricted driver's license
  • Prior computer experience and basic computer fluency
