Case Manager - SEED

Pay is commensurate with experience within a range of $20.00 to 27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility of the Supported Employment/Education Case Manager (SEED) will be the development and delivery of a comprehensive supported employment /education program for adults who have significant barriers in the vocational or educational area, primarily recurrent mental illness. Specific duties include but not limited to:

  • providing access to a diverse array of community vocational, educational, and volunteer opportunities and matching these opportunities with the clients skills, strengths, and interests in mainstream/normalized settings;
  • developing and monitoring traditional and non-traditional community vocational , educational and volunteer opportunities for clients and providing specialized follow-up for each client after establishing the client in a work setting;
  • providing one-on-one support and on the job coaching and training as needed to maintain the client in normalized vocational settings;
  • developing and maintaining effective employer relations and integrated job opportunities;
  • conducting psychosocial group activities in conjunction with other staff as needed: and
  • supervising and coordinating pre-vocational work units in the psychosocial work program.

Qualifications: The SEED Case Manager position requires but not limited to:

  • a bachelor's degree in psychology, social work, business, human development, counseling, occupational or recreational therapy, or a related field;
  • prefer work experience in the business community or experience providing rehabilitation services for disadvantaged populations
  • reliable transportation and a valid, unrestricted driver's license;
  • prior computer experience and basic computer fluency;
  • flexibility in assigned working hours;
  • a commitment to interdisciplinary and community mental health center approaches; and
  • an energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.
  • must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record;
