Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities:  The primary responsibility of the CPST Case Manager for adults who meet the criteria for the targeted population includes these specific duties but not limited to:

• To help clients to understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives.
• Assist clients in learning and practicing news skills that contribute to their recovery as specified in the treatment plan.
• Provide supportive counseling, solutions focused interventions; behavioral analysis to increase clients ability to function in all life domains.
• Providing outreach and crisis intervention services to clients and prospective clients to enable timely access to those services appropriate and necessary for crisis stabilization with the purpose of diverting the client from hospitalization and allowing the client to maintain independent living;
• Complete strength assessment with client and participate in treatment planning and review with CPST Manager.
• Additional responsibilities that may be assigned as circumstances warrant.
• Regular and predictable attendance is an essential function of the job.
• Flexibility to provide after hour coverage for crisis services.

Qualifications:  The CPST Case Manager position requires but not limited to:

• a BA/BS degree or four years of equivalent education and/or experience working in the human services field.
• reliable transportation; and a valid, unrestricted driver's license
• prior computer experience and basic computer fluency;
• must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record;
• an ability to work independently and in cooperation with others;
• flexibility in assigned working hours;
• a commitment to interdisciplinary and community mental health center approaches; and
• an energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.
