Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibilities of the Case Manager with the ACT team are providing NFMH Liaison services and community-based, Case Management services, within the trans-disciplinary team, according to the KDADS CMHC/CCBHC fidelity standards. These duties encompass but not limited to:
- Serving as a participating member on a community-based, trans-disciplinary, supportive service team operating according to KDADS CCBHC;
- Partnering with program staff to ensure service quality standards of the program, and specifically that fidelity standards are met; and generally, that data collection, documentation, reporting, outcomes standards, evidence-based standards, and deadlines are met;
- Coordinating and cooperating with team members and clientele to formulate and implement strategies for addressing consumer housing, mental health, substance use treatment, and primary health service needs;
- Acquiring a working knowledge of the other specialized roles on the ACT team and working with clients to provide services associated with those roles as needed;
- Coordinating community-based activities for clients with persistent mental health concerns to assist them in securing access to and mastering the use of community services and resources that will both reduce or eliminate the risk of hospitalization and support independent living in the community;
- Regularly visit with every person admitted to the NFMH from the service area of the CMHC;
- Follow the rules of the NFMH facility while on or inside the NFMH premises or facility.
- Regular and predictable attendance is essential;
- Assuming additional responsibilities that may be assigned as circumstances warrant.
Qualifications: The Case Manager position requires but not limited to:
- Bachelor's degree in psychology, social work, or related field and significant experience in the area of housing, mental health services, homeless services; or 2+ years of direct experience as a service provider in mental health, homelessness, housing, and SUD treatment services;
- Knowledge of community service programs and resources designed to assist in targeted populations of the county;
- A commitment to working with parents, family members, and consumers as partners;
- Knowledge of or experience in community mental health;
- Reliable transportation;
- Prior computer experience and administrative level computer fluency;
- Flexibility in assigned working hours;
- An energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.
