Responsibilities:  

The primary responsibility of the Case Manager-Community Based Services will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families.  

Some specific duties include:

  • Providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;
  • Delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis;
  • Providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;
  • Providing services on an as-needed basis both with respect to time-of-day and location;
  • Delivering reimbursable hours of service compliant with established standards;
  • Completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or the Center's operations in a timely and accurate manner;
  • Assisting with the evolution of an effective, efficient program of services; and
  • Responding to crises while on call in rotation with other similarly trained staff.

 Qualifications

  • A BA/BS degree or four years of equivalent education and/or experience working in the     human services field.
  • Reliable transportation and automobile insurance; and a valid, unrestricted driver's license;
  • Prior computer experience and basic computer fluency;
