The primary responsibility
of the Case Manager-Community Based Services will be case management
services, resource acquisition, crisis intervention, and outreach activities to
specified youth and their families.
Some specific duties include:
case management services, resource acquisition, crisis intervention, outreach
activities and coordinate these services with other community health and social
services in a variety of locations, including but not limited to the client's
home, the school, the Child & Family Services program site, or another
community setting, on an as-needed basis;
transportation to and from various activities or appointments as indicated by
the treatment plan or as instructed by the QMHP;
services on an as-needed basis both with respect to time-of-day and location;
reimbursable hours of service compliant with established standards;
all paperwork associated with the financial, clinical, legal, or regulatory
management of the case or the Center's operations in a timely and accurate
with the evolution of an effective, efficient program of services; and
to crises while on call in rotation with other similarly trained staff.
- A BA/BS degree or four
years of equivalent education and/or experience working in the human
transportation and automobile insurance; and a valid, unrestricted
computer experience and basic computer fluency;
