Responsibilities 

Under the supervision of the Director, Integrated Crisis Team (ICT), the ICT Crisis Case Manager (CCM) provides short term transitional community, center, and hospital based crisis support services and crisis case management services to children, adolescents, and adults through a multidisciplinary collaborative service approach.  The CCM will be comfortable with the pace of emergency room care, medical acute inpatient hospital care, and Community Mental Health Access to Care.

Some specific duties include:

  • Provide short-term transitional community, mental health center, and hospital based crisis case management and support to clients who present for services primarily related to mental health and substance use disorder within the LMH Health and Community Mental Health Center Setting until a case manager is assigned or existing case manager can be connectedTo help crisis clients to understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives and to develop an individualized crisis plan with client.
  • Provide 1:1 support for crisis clients awaiting inpatient hospitalization.
  • Provides crisis support services through implementation of evidenced based practices for children, adolescents, and adults who need mental health services.
  • Coordinate all integrated out-patient care needs and services and make appropriate referrals and follow up as needed.
  • Supports consultation to clients, families, and staff regarding care, intervention, prevention, and outpatient treatment recommendations.
  • Provides timely feedback regarding care, treatment recommendations and progress of patients via documentation in client charts, and through verbal feedback.
  • Assists clients by promoting engagement in treatment.

Qualifications: 

  • BA/BS Degree or four years of equivalent education and/or experience working in the human service field.
  • Reliable transportation; and a valid, unrestricted driver's license and proof of current car insurance.
  • Prior computer experience and basic computer fluency
  • his position operates primarily within the LMH Health Emergency Department and all recognized holidays and leave are in the accordance with LMH Health leave schedule.
