Integrated Dual Diagnostic Treatment Case Manager

Pay is based on experience from $20.00-$23.00 per hour. This is a full-time, hourly, non-exempt and benefit-eligible position.  Bert Nash offers excellent benefits which include medical, dental, vision, KPERS retirement, life insurance, an Employee Assistance Program (EAP) providing free counseling and resources, and generous PTO.

Responsibilities:

The primary responsibility of the IDDT Case Manager will be the provision of case management services to adults with recurrent mental health and substance abuse disorders. 

Specific duties include but are not limited to:

  • Conducting goal-directed community-based activities for adults with recurrent mental health and substance abuse disorders to assist the client in securing access to and mastering the use of community services and resources that will both reduce or eliminate the risk of hospitalization and support independent living in the community;
  • Coordinating the services listed above with other case managers, IOP staff, acute care staff, Access Center staff, medication services staff, homeless case management staff, and other community providers by communicating with such staff and community providers on a regular basis and coordinating schedules and assignments;
  • Providing outreach and crisis intervention services to clients and prospective clients to enable timely access to those services appropriate and necessary for crisis stabilization with the purpose of diverting the client from hospitalization and allowing the client to maintain independent living;
  • Delivering reimbursable hours of service compliant with established BNC standards;
  • Completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or BNC's operations in a timely and accurate manner;
  • Participating in public information and public relations activities, serve as a community liaison and advocate for mental health and the Bert Nash Center; and
  • Providing assistance with the evolution of an effective, efficient preventative program of services.
  • Regular and predictable attendance is an essential function of the job

Qualifications:

Including but not limited to:

  • BA/BS degree or four years of equivalent education and/or experience working in the human services field;
  • Reliable transportation; and a valid, unrestricted driver's license;
  • Prior computer experience and basic computer fluency;
  • Must be able to achieve within one's first 6 months of employment a working  competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record;
  • An ability to work independently and in cooperation with others;
  • Ability to exercise sound independent judgment;
  • Strong organizational skills;
  • Strong oral and written communication skills;
  • Strong interpersonal skills in working with diverse publics;
  • Flexibility in assigned working hours;
  • A commitment to interdisciplinary and community mental health center approaches; and
  • An energetic and positive approach to the rapidly evolving changes and challenges of a complex work place. 

Bert Nash Center requires proof of Covid-19 immunization prior to starting any position. Covid-19 vaccine series must be completed or in process to be cleared for new hire orientation.
