Responsibilities: The main responsibilities of the Integrated Care Nurse Coordinator are to assist the Integrated Care Director and the team with the operational, programmatic and personnel activities to support the primary care needs of Bert Nash Center patients, to support population health and integrated care model implementation. This position will provide direction and leadership to the Integrated Care Program and has primary responsibility for implementing, managing and evaluating the Integrated Care program and medical services nursing staff. Specific duties will include:

  • Organize and lead the delivery of integrated care for patients of the Bert Nash Center.
  • Lead the day-to-day operations and coordination of the integrated care program.
  • Supervise and serve as a clinical role model and resource for medical services nursing staff and other healthcare support staff within Medical Services as assigned.
  • Lead the implementation and monitoring of designated program standards and outcomes to identify opportunities for program improvement and to ensure program effectiveness.
  • Collaborate to build and maintain accurate, comprehensive and current program documentation tools in the electronic medical record (EMR).
  • Assume additional responsibilities that may be assigned as circumstances warrant.

Qualifications: The Integrated Care Nurse Coordinator position requires:

  • Bachelor's in nursing, but Master's degree in healthcare management is preferred.
  • Current, active Kansas nursing license.
  • Minimum of three years' work experience in a managerial role within primary care or mental health with knowledge of health care systems is accepted, but work experiences showing progressive experience in clinical program development and management is preferred.
  • Knowledge of psychopharmacological interactions
  • Readiness/ability to exercise leadership in the continuing process fundamental to the Center's successful adaptation to a dynamic environment
