Crisis Case Manager - Part time - Treatment & Recovery Center

Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Part time, non-exempt, not benefits eligible.

Responsibilities: Under the supervision of the Director, Integrated Crisis Team (ICT), the ICT Crisis Case Manager (CCM) provides short term transitional community, center, and hospital based crisis support services and crisis case management services to children, adolescents, and adults through a multidisciplinary collaborative service approach. The CCM will be comfortable with the pace of emergency room care, medical acute inpatient hospital care, and Community Mental Health Access to Care.
Specific duties include but not limited to:

  • Provide short-term transitional community, mental health center, and hospital based crisis case management and support to clients who present for services primarily related to mental health and substance use disorder within the LMH Health and Community Mental Health Center Setting until a case manager is assigned or existing case manager can be connected. To help crisis clients to understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives and to develop an individualized crisis plan with client.
  • Provide 1:1 support for crisis clients awaiting inpatient hospitalization.
  • Provides crisis support services through implementation of evidenced based practices for children, adolescents, and adults who need mental health services.
  • Coordinate all integrated out-patient care needs and services and make appropriate referrals and follow up as needed.
  • Supports consultation to clients, families, and staff regarding care, intervention, prevention, and outpatient treatment recommendations.
  • Delivering reimbursable hours of service compliant with established Bert Nash Center standards when appropriate.
  • Completes all paperwork and electronic medical record (EMR) entry associated with interventions provided.
  • Other duties as assigned.


Qualifications: The ICT Crisis Case Manager position requires but not limited to:

  • BA/BS Degree or four years of equivalent education and/or experience working in the human service field.
  • Reliable transportation; and a valid, unrestricted driver's license and proof of current car insurance.
  • Prior computer experience and basic computer fluency
  • Must be able to achieve within one's first 6 months of employment a working competency in the management and use of Centers electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record.
  • An ability to work independently and in cooperative with others.
  • This position operates primarily within the LMH Health Emergency Department and all recognized holidays and leave are in the accordance with LMH Health leave schedule.
