Mental Health Center (CMHC) of Douglas County is to advance the health of the
community through comprehensive behavioral health services responsive to

Pay is
based on experience starting at $20.00-$23.00 per hour, Full Time - 40 hours
per week, non-exempt, benefits eligible. Bert Nash offers excellent benefits

Responsibilities:
The
primary responsibility of the Community Support Services Case Manager - Strengths Team is to coordinate case management services for adults in the
community with recurrent mental health issues, develop individualized treatment
goals, and aid in accessing treatment.
Specific
duties include but are not limited to:
- Helping clients
understand the symptoms of their mental illness and to minimize the effect
of those symptoms in all the significant areas of their lives;
- Assisting clients
in learning and practicing new skills that contribute to their recovery as
specified in the treatment plan;
- Providing supportive
counseling, solutions focused interventions; behavioral analysis to
increase client's ability to function in all life domains;
- Providing
outreach and crisis intervention services to clients and prospective
clients to enable timely access to those services appropriate and
necessary for crisis stabilization with the purpose of diverting the
client from hospitalization and allowing the client to maintain
independent living;
- Completing
strength assessment with client and identify strategies or treatment
options associated with the consumer's mental illness with the goal of
minimizing the negative effects of mental illness symptoms or associated
environmental stressors;
- Developing individualized
crisis plan with client and review during treatment plan review time;
- Coordinating
all clients' services with other providers, family, and community
collaterals primarily with the client consulting in the process;
- Delivering
reimbursable hours of service compliant with established BNC standards
- Completing all
paperwork associated with the financial, clinical, legal or regulatory
management of the case of BNC's operations in a timely and accurate
manner;
- Participating
in public information and public relations activities, serve as a
community liaison and advocate for mental health and the Bert Nash Center;
- Providing
assistance with the evolution of an effective, efficient preventative
program of services;
- Additional
responsibilities that may be assigned as circumstances warrant
specifically related to supportive housing facilities and clients; and
- Regular and
predictable attendance is an essential function of the job.
Qualifications:
Including
but not limited to:
- BA/BS degree or
four years of equivalent education and/or experience working in the human
services field;
- Reliable
transportation; and a valid, unrestricted driver's license and proof of
current car insurance;
- Prior computer
experience and basic computer fluency;
- Must be able to
achieve within one's first 6 months of employment a working competency in
the management and use of the Center's electronic medical record (EMR)
software as demonstrated by timely and accurate documentation of the
clinical record;
- An ability to
work independently and in cooperation with others;
- Ability to
exercise sound independent judgment;
- Strong
organizational skills;
- Strong
interpersonal skills in working with diverse publics;
- A commitment to
interdisciplinary and community mental health center approaches; and
- An energetic
and positive approach to the rapidly evolving changes and challenges of a
complex workplace.
Bert Nash Center requires
proof of Covid-19 immunization prior to starting any position. Covid-19 vaccine
series must be completed or in process to be cleared for new hire orientation.
