Case Manager - SOAR

Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: Case management through SOAR provides support to adults with mental illness for the purpose of expediting the SSI/SSDI and Medical benefits applications process. This position reports to the SOAR Program Manager and duties include, but are not limited to the following:

  • Collaborate with clients to complete SSI/SSDI, KanCare, and Department of Children and Families applications according to established policies and procedures of the program;
  • Meet with clients at least once a week during the application process to ensure timely completion.
  • Link consumers to Social Security Administration, KanCare, and Department of Children and Families to coordinate information and needed documents between Social Security Administration and Disability Determination Services;
  • Review clients that are on the Fee Waiver to see if they are eligible for the SOAR process according to established policies and procedures (this responsibility will rotate monthly to different members of the SOAR team)
  • Complete at least 5 cases per year, maintain a caseload of 3-5 cases at a time and complete a SOAR claim with clients within a 60-day timeframe as per SOAR program manager's discretion
  • Work with the SOAR team to vet and triage candidate cases, and assign and take on a new case as the team determines is necessary
  • Provision of additional responsibilities as circumstances warrant; and
  • Regular and predictable attendance is an essential function of the job.

Qualifications:  The SOAR Case Manager requires but not limited to:

  • a BA/BS degree or four years of equivalent education and/or experience working in the human services field.
  • reliable transportation, and a valid, unrestricted drivers license;
  • completion and obtainment of SAMHSA SOAR certification within 45 days of hire as a Bert Nash SOAR provider
  • completion and obtainment of KDHE Presumptive Eligibility training within 6 months of hire
  • exposure and knowledge of SSI/SSDI and other social services programs
  • occasional flexibility with work schedule;
  • ability to set and adhere to boundaries;
  • commitment to interdisciplinary and community mental health center approaches;
  • an energetic and positive approach to the rapidly evolving changes and challenges of a complex work place;
