Case Manager - Housing

Pay is commensurate with experience within a range of $20.00 to 27.68 per hour. Full time, non-exempt, benefits eligible. Responsibilities: include but not limited to: The primary responsibility of the Supportive Housing Case Manager (CPST) is to work with people (mainly adults) whom are currently struggling with symptoms of mental illness, that also need assistance with obtaining and maintaining independent community housing.  The consumers are served by the Bert Nash Community Mental Health Center.

  • To help clients to understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives;
  • Complete strength assessments, collaborate in treatment planning and review the treatment plan with the client;
  • Assist clients in learning and practicing news skills that contribute to their recovery as specified in their individualized treatment plan;
  • Provide supportive counseling, solutions focused interventions; behavioral analysis to increase client's ability to function in all life domains;
  • Complete individualized assessments with clients and identify strategies or treatment options associated with the consumer's mental illness with the goal of minimizing the negative effects of mental illness symptoms or associated environmental stressors;
  • Additional responsibilities that may be assigned as circumstances warrant;
  • Flexibility to provide some after hour assistance clients in accessing the different locations within the Bert Nash CMHC network of services;
  • Regular and predictable attendance is an essential function of the job. 

Qualifications:  The Supportive Housing Case Manager position requires but not limited to:

  • BA/BS degree (psychology, social work, or psychiatric nursing preferred) and/or four years of equivalent experience working in the human services field;
  • Ability to relate and effectively provide services to people who are experiencing mental health and housing related crisis;
  • Experience working with individuals who are experiencing a serious and persistent mental illness, have a history of substance use and might have experienced significant trauma;
  • Prior computer experience and basic computer fluency;
  • Must be able to lift 25 lbs or more to assist the client in ADLs, IADLs;
  • Must have reliable transportation and valid, unrestricted driver's license;
  • Commitment in providing services with a Trauma Informed and Person-Centered approach;
  • Flexibility in assigned working hours.
