Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility of the Case Manager-Strengths is to coordinate case management services for adults in the community with recurrent mental health issues, develop individualized treatment goals, and aid in accessing treatment. Specific duties include but not limited to:
- To help clients to understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives.
- Assist clients in learning and practicing new skills that contribute to their recovery as specified in the treatment plan.
- Provide supportive counseling, solutions focused interventions; behavioral analysis to increase client's ability to function in all life domains.
- Providing outreach and crisis intervention services to clients and prospective clients to enable timely access to those services appropriate and necessary for crisis stabilization with the purpose of diverting the client from hospitalization and allowing the client to maintain independent living;
- Coordinate all clients' services with other providers, family, and community collaterals primarily with the client consulting in the process.
- Completing all paperwork associated with the financial, clinical, legal or regulatory management of the case of BNC's operations in a timely and accurate manner
- Additional responsibilities that may be assigned as circumstances warrant specifically related to supportive housing facilities and clients; and
- Regular and predictable attendance is an essential function of the job.
Qualifications: The Case Manage-Strengths position requires but not limited to:
- BA/BS degree or four years of equivalent education and/or experience working in the human services field.
- Reliable transportation; and a valid, unrestricted driver's license and proof of current car insurance
- Prior computer experience and basic computer fluency.
- An ability to work independently and in cooperation with others.
- Ability to exercise sound independent judgment
