Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility
of the Child & Family Services Case Manager will be case management
services, resource acquisition, crisis intervention, and outreach activities to
specified youth and their families.
Specific duties include but not limited to:
- Providing case management services,
resource acquisition, crisis intervention, outreach activities and coordinate
these services with other community health and social service agencies;
- Delivering services in a variety of
locations, including but not limited to the client's home, the school, the
Child & Family Services program site, or another community setting, on an
as-needed basis;
- Providing
transportation to and from various activities or appointments as indicated by
the treatment plan or as instructed by the QMHP;
- Responding to
crises while on call in rotation with other similarly trained staff.
- To be successful
in this position late evenings and/or weekend hours are required to meet the
needs of our clients and their families
- Regular and predictable attendance is an essential function of the job.
- Case manager is expected to meet regularly with each QMHP
directing the treatment plans for their clients at
least once monthly to review cases and ensure direction of clinical services.
- Case manager is expected to contact a QMHP if urgent clinical
consultation is required. Case managers will be notified of who the available
QMHP is for each week.
- Additional responsibilities may be
assigned as circumstances warrant.
Qualifications: The Child & Family Services Case Manager
position requires but not limited to:
- BA/BS degree or four
years of equivalent education and/or experience working in the human
services field.
- Reliable
transportation and automobile insurance; and a valid, unrestricted
driver's license;
- Prior
computer experience and basic computer fluency;
- Must be able to achieve within one's
first 6 months of employment a working competency in the management and use of
the Center's electronic medical record (EMR) software as demonstrated by timely
and accurate documentation of the clinical record;
- Demonstrated
strong interpersonal skills in working with diverse publics, consumers, and
referral agents;
- Flexibility
in assigned working hours;
- A commitment to interdisciplinary and community mental health center approaches;
and
- An
energetic and positive approach to the rapidly evolving changes and challenges
of a complex workplace.
