Case Manager - CFS

Pay is commensurate with experience within a range of $20.00-$24.00   This position is full-time, non-exempt and eligible for our benefits. A. Responsibilities and Duties:  The primary responsibility of the Child & Family Services Case Manager will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families.  Specific duties include:

1. providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;

2. delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis;

3. providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;

4. providing services on an as-needed basis both with respect to time-of-day and location;

5. delivering reimbursable hours of service compliant with established standards;

6. completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or the Center's operations in a timely and accurate manner;

7. assisting with the evolution of an effective, efficient program of services; and

8. responding to crises while ?on call? in rotation with other similarly trained staff.

9. To be successful in this position late evenings and/or weekend hours are required to meet the needs of our clients and their families

10. regular and predictable attendance is an essential function of the job.

11. Case manager is expected to meet regularly with each QMHP directing   the treatment plans for their clients at least once monthly to review cases and ensure direction of clinical services.

12. Case manager is expected to contact a QMHP if urgent clinical consultation is required. Case managers will be notified of who the available QMHP is for each week.

13. Additional responsibilities may be assigned as circumstances warrant.

B. Accountability:  The Child & Family Services Case Manager will be directly accountable to the Child & Family Services Program Manager or his/her designee.  The Child & Family Services Program Manager or his/her designee will, no less than annually, complete a performance evaluation in compliance with personnel policies.

C. Qualifications:  The Child & Family Services Case Manager position requires:

1.  a BA/BS degree or four years of equivalent education and/or experience working in the human services field.

2.  reliable transportation and automobile insurance; and a valid, unrestricted driver's license;

3.  prior computer experience and basic computer fluency;

4.  must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record;

5.  an ability to work independently and in cooperation with others;

6.  strong organizational skills;

7.  strong oral and written communication skills;

8.  demonstrated strong interpersonal skills in working with diverse publics, consumers, and referral agents;

9.  flexibility in assigned working hours;

10.  a commitment to interdisciplinary and community mental health center approaches; and

11.  an energetic and positive approach to the rapidly evolving changes and challenges of a complex workplace.

Bert Nash Center requires proof of Covid-19 immunization prior to starting any position. Covid-19 vaccine series must be completed or in process to be cleared for new hire orientation.
